Active substance

Prednisone (Prednison) — a synthetic glucocorticoid, a prodrug: in the liver it is converted into the active metabolite prednisolone. ATTENTION: in Decortin (without «H») the active substance is specifically prednisONE (ATC H02AB07). This is NOT the same as Decortin H, in which the active substance is prednisoLONE (ATC H02AB06).

Available forms

Tablets. Decortin (without H) is available in strengths of 5 mg, 20 mg and 50 mg (some pack sizes have been withdrawn from sale). For comparison, the Decortin H (prednisolone) range includes 1 / 5 / 10 / 20 / 50 mg, as well as Solu-Decortin H for injection. Manufacturer — Merck (Merck Serono GmbH, Darmstadt).

Indications

  • Inflammatory and rheumatic diseases (rheumatoid arthritis, systemic lupus erythematosus)
  • Autoimmune diseases (relapses of multiple sclerosis, autoimmune hepatitis)
  • Severe allergic reactions (severe allergic rhinitis, bronchial asthma)
  • Skin diseases (severe eczema, psoriasis)
  • Inflammatory bowel diseases (Crohn’s disease, ulcerative colitis)
  • Blood diseases and severe respiratory exacerbations (certain anaemias, COPD exacerbations)

How to take

The dose and duration are always determined by the doctor — the regimen is strictly individual and depends on the disease and the response to treatment. The entire daily dose is usually taken once in the morning (corresponding to the natural circadian rhythm of cortisol production). Maintenance doses for chronic conditions are, as a rule, 5–15 mg per day. The medicine must not be stopped abruptly: the dose is reduced gradually, in steps (below 30 mg/day — in smaller steps).

Contraindications

  • Known hypersensitivity (allergy) to prednisone or components of the medicine
  • Acute viral, bacterial and fungal infections without cover by adequate therapy
  • Active or latent infections (for example, tuberculosis, hepatitis B) — risk of reactivation
  • Administration of live vaccines during immunosuppressive doses
  • Prescribed with caution in diabetes mellitus, arterial hypertension, osteoporosis, peptic ulcer disease of the GI tract, glaucoma, mental disorders — the decision is made by the doctor, weighing benefit and risk

Possible side effects

  • Increased susceptibility to infections (immune suppression)
  • Suppression of adrenal function, Cushingoid syndrome
  • Increased blood sugar, metabolic disorders, weight gain
  • Osteoporosis, muscle weakness, delayed wound healing
  • GI ulcers and bleeding
  • Mental and neurological disturbances (mood swings, insomnia)
  • Ophthalmological complications (cataract, glaucoma), growth retardation in children

Special precautions

Prescription medicine (verschreibungspflichtig) — dispensed only as prescribed by a doctor. Treatment must not be stopped abruptly: after long-term use the dose is reduced gradually (risk of adrenal insufficiency). Avoid live vaccines in the period from 8 weeks before to 2 weeks after vaccination. IMPORTANT DISTINCTION: Decortin (without «H») contains prednisONE (Prednison, ATC H02AB07), whereas Decortin H contains prednisoLONE (Prednisolon, ATC H02AB06). These are two different, although related, active substances: prednisone is converted into prednisolone in the liver. Do not confuse the medicines and do not substitute one for the other without a doctor’s instruction.
