Active substance

Chlortalidone (Chlortalidon) — a long-acting thiazide-like diuretic

Indications

  • Arterial hypertension (high blood pressure)
  • Oedema of cardiac, hepatic and renal origin
  • Marked (manifest) heart failure
  • Nephrogenic diabetes insipidus (when other medicines are unsuitable)

How to take

In hypertension: per the instructions a starting dose of 12.5–50 mg of chlortalidone once a day (in the morning, with breakfast), maintenance dose — 12.5–25 mg per day. In oedema: a starting dose of 50–100 mg/day, maintenance 25–50 mg/day; the maximum daily dose is 200 mg (higher doses do not increase the effect). The exact dose and duration of treatment are determined only by the doctor.

Contraindications

  • Hypersensitivity to chlortalidone, thiazides and sulfonamide derivatives
  • Anuria, severe renal failure (creatinine clearance <30 ml/min), glomerulonephritis
  • Severe hepatic impairment
  • Treatment-resistant hypokalaemia, severe hyponatraemia, hypercalcaemia
  • Symptomatic hyperuricaemia (gout)
  • Pregnancy (use is contraindicated), breastfeeding (must not be used while breastfeeding — or breastfeeding should be stopped)
  • Children and adolescents under 18 years of age

Possible side effects

  • Hypokalaemia (decreased potassium — very common) and other electrolyte disturbances
  • Hyperuricaemia — gout attacks may occur in predisposed individuals
  • Increased cholesterol and triglyceride levels
  • Lowering of blood pressure, dizziness
  • Gastrointestinal disturbances (nausea, vomiting)
  • Muscle weakness
  • Increased skin sensitivity to ultraviolet light

Special precautions

  • Prescription medicine in Germany (verschreibungspflichtig)
  • Regular monitoring of blood electrolytes is required — primarily potassium, sodium, magnesium and calcium
  • Special monitoring in patients with diabetes mellitus and gout
  • During treatment, adequate fluid intake and potassium-rich foods are recommended; avoid liquorice
  • Contains lactose (lactose monohydrate 32.58 mg in the 25 mg tablet)
