Active substance
Salbutamol (as salbutamol sulfate) — a short-acting selective beta2-agonist (SABA, ATC R03AC02). It relaxes the bronchial muscles and rapidly widens the narrowed airways.
Available forms
In Germany it is manufactured by GlaxoSmithKline: the metered-dose aerosol Sultanol Dosier-Aerosol, 100 mcg of salbutamol per inhalation (canister of 200 doses). There are also nebuliser forms: the solution Sultanol Lösung für einen Vernebler (Inhalationslösung) 5 mg/ml in a 10 ml bottle, as well as the ready-to-use dilutions Sultanol Fertiginhalat 1.25 mg/2.5 ml and Sultanol forte Fertiginhalat 2.5 mg/2.5 ml.
Indications
- Relief of an acute attack of breathlessness in bronchial asthma
- Symptomatic treatment of paroxysmal shortness of breath in bronchial asthma (reversible airway obstruction)
- COPD — chronic obstructive pulmonary disease with a reversible obstructive component (as part of combination therapy, as a reliever)
- Prevention of bronchospasm induced by physical exertion or contact with an allergen
How to take
According to the official instructions, for an acute attack adults and adolescents from 12 years take 1–2 inhalations (100–200 mcg of salbutamol), and children aged 4–11 years take 1 inhalation (100 mcg). To prevent bronchospasm caused by physical exertion or contact with an allergen — 1–2 inhalations 10–15 minutes before exertion. For maintenance use — 1–2 inhalations 3–4 times a day; a maximum of 8 inhalations per day. Shake the canister before use. The dose and frequency are determined by the doctor. If the usual dose has stopped helping, relief does not occur, or shortness of breath worsens after inhalation, seek medical advice immediately.
Contraindications
- Hypersensitivity to salbutamol or the medicine’s components
- Concurrent use of beta-blockers (beta-adrenergic blocking agents)
Possible side effects
- Tremor (muscle trembling, especially of the hands)
- Headache
- Tachycardia, palpitations, cardiac rhythm disturbances
- Nausea
- Restlessness, inner tension
- Muscle cramps; at high doses — a decrease in blood potassium (hypokalaemia)
Special precautions
Prescription medicine — in Germany it is dispensed only on a doctor’s prescription. Sultanol is a symptomatic reliever: it relieves an attack but does not treat inflammation and does not replace the basic (anti-inflammatory) therapy of asthma. An increasing need for inhalations is a sign of worsening asthma control: you must consult a doctor to review treatment. If shortness of breath worsens after inhalation, or a previously effective dose no longer brings relief, urgent medical care is required. Use with caution and only on strict indications in the following conditions: tachycardic cardiac rhythm disturbances (tachyarrhythmias), hypertrophic obstructive cardiomyopathy; recent myocardial infarction, severe coronary heart disease; severe uncontrolled arterial hypertension; hyperthyroidism (overactive thyroid), phaeochromocytoma, aneurysms. Also use with caution when taking cardiac (digitalis) glycosides and in poorly controlled diabetes mellitus.
