Active substance

In Germany it is manufactured by Pfizer Pharma GmbH (Berlin): Tavor tablets 0.5 mg, 1.0 mg and 2.5 mg; Tavor Tabs tablets 2.0 mg; Tavor Expidet 1.0 mg and 2.5 mg — lyophilisate wafers that dissolve in the mouth without water.
Indications
- Symptomatic short-term treatment of feelings of tension, agitation and anxiety
- Sleep disturbances — only severe ones not remediable by other measures (when a sedative effect is also desirable during the daytime)
- Sedation (premedication) before operations and diagnostic interventions, as well as after them
How to take
According to the official prescribing information, the usual daily dose of lorazepam for adults is 0.5–2.5 mg, divided into 2–3 intakes (or once in the evening). For sleep disturbances, the entire daily dose (0.5–2.5 mg) is taken once, about half an hour before sleep. In individual cases, especially in a hospital setting, the doctor may increase the dose — no more than 7.5 mg/day. For elderly and debilitated patients, as well as in cases of impaired liver and kidney function, the dose is reduced — usually by half. The medicine is intended only for a short course: in acute conditions these are single intakes or a few days; after 2 weeks of daily intake, the doctor, while gradually reducing the dose, decides whether further therapy is needed. The dose and duration of treatment are determined solely by the doctor. Intake must not be stopped abruptly — withdrawal is carried out gradually according to the doctor’s schedule.
Contraindications
- Hypersensitivity to lorazepam, other benzodiazepines or components of the medicine
- Dependence on alcohol, medicines or drugs — including in the past
- Sleep apnoea syndrome
- Severe respiratory insufficiency, chronic obstructive pulmonary diseases (only at the doctor’s decision)
- Myasthenia (myasthenia gravis)
- Acute poisoning with alcohol or medicinal products
- Pregnancy and the breastfeeding period (lorazepam passes into breast milk and may cause sedation and weak sucking in the infant); not used in children under 6 years, and under 18 years — only for special indications

Special precautions
Prescription medicine — in Germany dispensed only on a doctor’s prescription. Lorazepam therapy carries a high risk of developing physical and psychological dependence even with regular intake over a short time, so the minimum effective dose is used for the shortest possible course. With abrupt discontinuation, withdrawal syndrome is possible — the medicine should be discontinued gradually under a doctor’s supervision. During treatment, the consumption of alcohol is not permissible. The medicine slows reactions: during therapy you should refrain from driving a car and operating machinery. With long-term use, the development of tolerance (weakening of the effect) is possible.
