Indications
- Arterial hypertension (elevated blood pressure)
- Angina pectoris / coronary heart disease; prevention of recurrent myocardial infarction
- Tachycardic cardiac rhythm disturbances (arrhythmias with a rapid pulse)
- Prevention of migraine attacks
- Essential tremor (trembling)
- Symptomatic treatment of hyperthyroidism/thyrotoxicosis (palpitations, etc.) and functional cardiac complaints (hyperkinetic heart syndrome)
How to take
The dose and duration of treatment are determined by the doctor on an individual basis. Approximate doses per the Fachinformation: hypertension — a starting dose of 80–120 mg/day, if necessary 160–240 mg/day (maximum 320 mg/day); angina and arrhythmias — usually 40 mg 3 times a day; prevention of migraine and essential tremor — 80–120 mg/day; hyperthyroidism — 30–160 mg/day in 3–4 divided doses; functional cardiac complaints — 10–40 mg 3 times a day (Dociton 10 mg). The tablets are taken before meals, without chewing, with liquid. The medicine must not be stopped abruptly — the dose is reduced gradually under the doctor’s supervision.
Contraindications
- Bronchial asthma and COPD (a tendency to bronchospasm and attacks of breathlessness)
- AV block of II–III degree, sick sinus syndrome
- Marked bradycardia (resting pulse below 50 bpm)
- Untreated/decompensated heart failure
- Marked hypotension (low blood pressure), shock, acidosis
- Severe peripheral circulatory disorders
- Untreated phaeochromocytoma; hypersensitivity to propranolol
Possible side effects
- Bradycardia (slowing of the pulse), palpitations
- Fatigue, weakness
- Dizziness, headache
- Gastrointestinal disturbances (nausea, vomiting, constipation)
- Sleep disturbances, sometimes low mood
- A feeling of coldness in the extremities
- Excessive drop in blood pressure
Special precautions
- Prescription medicine (verschreibungspflichtig) — dispensed only on a doctor’s prescription
- It must not be stopped abruptly: rebound tachycardia and worsening of angina may occur; discontinuation should be by gradual dose reduction
- Caution in diabetes mellitus: propranolol may mask the symptoms of hypoglycaemia (rapid heartbeat, trembling); regular blood sugar monitoring is required
- It may reduce reaction speed (driving, operating machinery)
